What is Semaglutide?
Semaglutide is a medication that acts like the hormone glucagon-like peptide-1 (GLP-1). You take it as a weekly injection under your skin.
GLP-1 increases your body’s production of insulin, which slows digestion.
When the stomach takes more time to empty itself of food, it sends signals of “fullness” to the brain. The end result of this process is a powerful appetite suppression effect based on the patient’s own natural insulin production.
Slow digestion also helps stabilize your blood sugar levels, giving you more energy throughout the day. As a result, you feel more energized, less fatigued and moody, and cravings are reduced.
Semaglutide is not a stimulant and doesn’t have the same side effects, such as agitation or a potentially higher risk of cardiovascular events.
What is Tirzepatide?
Tirzepatide works by affecting two hormones that are released after eating. The two hormones are GLP-1 and GIP by mimicking they way these two hormones work.
GLP-1 stands for glucagon-like peptide-1 and GIP stands for the glucose-dependent insulinotropic polypeptide. These hormones are involved in blood sugar control. In healthy people, these hormones message the pancreas to release insulin after eating and message the liver to lower the amount of glucose it’s producing. These instructions, when obeyed, make people feel full so they stop eating, which slows down digestion and lowers blood sugar.
Tirzepatide works by acting like these hormones and improves blood sugar control.
So, the drug helps the body release insulin when blood sugar is high, and helps the body remove excess sugar from the blood; it stops the liver from making and releasing too much sugar. In addition, it helps people to eat less.
Tirzepatide is more effective at lowering blood sugar and weight loss in type 2 diabetes patients than Semaglutide, an injected GLP-1 receptor agonist, or two commonly used forms of insulin when used in this setting.
Who Should Take These Medications?
Compounded Semaglutide and Tirzepatide can be prescribed to obese adults (with a BMI of at least 30) or overweight adults with a BMI of at least 27 and one or more weight-related health issues, not including diabetes.
You can choose compounded semaglutide and tirzepatide if diet and exercise alone aren’t working or if you are at risk of cardiovascular events (e.g., heart attacks or stroke).
How Well Do These Medications Work?
Clinical studies show that semaglutide is far more effective than diet and exercise alone in people experiencing difficulty losing weight.
In one clinical trial of 957 obese volunteers, injecting a 0.4mg dose of semaglutide led to an average weight loss of 13.8% over one year. Semaglutide was used together with diet and exercise, but these measures alone resulted in an average of 2.3% weight loss.
Another study found an average weight loss of 15% in the volunteers taking semaglutide. Almost 70% lost at least 10% of their body weight. If you weigh 300 pounds, 15% of your body weight is 45 pounds.
These effect sizes can prevent the need to turn to drastic weight loss surgery or stimulants with unpleasant or even dangerous side effects.
Contraindications
There are a few contraindications to taking GLP-1s in our weight loss program including:
family history or history of medullary thyroid cancer
diagnosis of Diabetes Type 1 or 2
significant kidney disease
pregnant or desiring to get pregnant in next 3 months (We require all female clients to commit to using some form of birth control OR have a plan in place to avoid pregnancy while on the these medications and for 2 months following completion )

How to get started with Medication Management

Step 1 - Schedule a Provider Consultation
We require a telehealth consultation with one of our Nurse Practitioners. This consultation will be approximately 20 minutes long. Our NP will review your health history, answer any questions you may have regarding medication and review our program in more depth.

Step 2 - Complete our Weight Loss Intake
Once you book your appointment, you will automatically receive an email inviting you to complete our Weight Loss Intake and Consent Forms. This is required to be completed at least 24 hours in advance of your appointment. Please check your inbox for this email (also check promotions and spam folders if you do not see it). If you are having any issues completing, please contact our office at 828-702-5321.

Step 3 - Phone Call with our Registered Nurse
Before your appointment, our Registered Nurse will contact you to discuss our program details, and prescreen for appropriateness of medication management based on the heath intake you completed. This is a great opportunity to share more about your goals and ask questions before your appointment!

Step 4 - Provider Visit & Labwork will be ordered
In order to serve you better and to make sure we are proving the best and safest care possible, We require the following bloodwork before your medication can be ordered.
CBC, CMP, Ferritin, Insulin, Hemoglobin A1C, Lipids, TSH, FT3 Free, FT4 Free, RT3, TPO ab, Tg ab, Homocysteine, Vitamin D.
This will be ordered through labcorp during your consultation appointment.

Step 5 - Schedule your Injection
Once we receive lab results and our Nurse Practitioner has reviewed, our team will will contact you to schedule your first injection appointment with our RN. This appointment will be 30 minutes. Our RN will get vital signs, conduct an assessement, provide education on medication, and administer your injection. Before you leave, our team will schedule your next 3 appointments. At each appointment, our RN will be providing additional resources and education to support you on your wellness journey!
